- Please do NOT return packages without completing this form and prior to having an RMA # issued to you. Your package could be refused or lost in our large facility if it is not labeled with an RMA #.
- Notes and requests submitted with a shipment may be missed or lost. We will only process requests submitted below. If you would like to change your request after submission, please email email@example.com.
- Return requests are answered in the order they are received. Our support staff are off duty on weekends. Please allow 1-3 business days for one of our friendly specialists to reach back to you regarding your RMA request.
- If you encounter a problem while submitting the request through this online form, you may also submit your RMA request with the same information via fax to (626) 285-8622 or via email to firstname.lastname@example.org.
Note: If you are unsure of your order number, you may log onto your account and click view order history to bring up all recent orders made. For the Evike.com Paid Repair Service, please put "Paid Repair" in the Order # field.
